Title: Myths and Misconceptions about Tuberculosis Transmission in Ghana

The validity of the instrument used for measuring misconception about TB should be discussed. By validity here is meant the degree to which a measurement measures the true value of the variable. Surely, the validity is not 100% for a measurement as subjective as the question on modes of spread of TB. Given this, some of the variation in the rates of misconception may be explained by differences in understanding the question properly, answering truthfully due to social circumstances surrounding the interaction between the surveyor and interviewed and simply not paying significant attention to those questions which are among many questions asked in the DHS surveys. In general, it can be discussed if one variable like this one can be picked out from a DHS survey and be analysed in great detail. There is relatively abundant literature from Macro systems Inc. (the company in charge of the DHS) on the validity of the DHS questions. The authors should discuss this literature and if possible also other studies on validity of household surveys, especially on knowledge and attitudes (KA in the KAP surveys). The variation in rates are in many instances on the smaller side and in some cases a bit confusing, such as when men in one region report low level of rejection of the five propositions made by the surveyor while women do not. This adds to the importance of discussing the validity of the survey question. Response: Questions about validity and related issues in KAP studies have been discussed as potential source of limitation to this study. The article is after revision now only focused on the questions on misconceptions of how TB is spread. It does not report data on correct knowledge on TB spread. It cannot be concluded that those who reject the five propositions made by the surveyors have good knowledge on how TB is spread. The only data we have in this version of the article are the respondents' answers to the question on five specific misconceptions. We cannot say anything about what understanding or beliefs on TB transmission that those who answered no to those questions had. The authors should review the article with this in mind.
